Appointment Cancellation Policy Agreement:
Future Health Management is committed to providing all of my clients with exceptional care. When a patient cancels without giving enough notice, they prevent another client from being seen.
Please call us 0208 991 1490 or 07956843575 giving me 48 hours’ notice prior to your scheduled appointment to notify me/clinic of any changes or cancellations. When booking a consultation, it is my policy to take half the consultation fee prior to the appointment, which will be non-refundable if you do not give between 48 to 24 hours’ notice.
- Over 48 hours prior to the appointment – no charge
- Between 48 hours and 24 hours – 50% of fee
- Less than 24 hours – 100% of the fee.
The terms will be emailed to you once the consultation has been booked and confirmed. I do ask clients to check their junk or spam filter. The completion of both the terms and nutritional questionnaire should be sent to me two days before the consultation.
If you are booking a Metabolic balance package, the initial consultation will be deducted from your metabolic balance program £1,200 – £180 = £1,020
This does not apply to any biofeedback package, if you choose a package then the fee of this package will be paid in full before the first consultation.
